How does Aldactide work?

This medicine contains two active ingredients hydroflumethiazide and spironolactone.Hydroflumethiazide acts in the kidney to remove excess water from the blood, by causing an increase in the removal of salts such as potassium and sodium. This removal of salts causes water to be drawn out of the blood and into the kidneys, where it is then excreted in the urine. Spironolactone achieves the same effect by blocking the actions of a hormone called aldosterone, which causes an increase in the removal of sodium from the blood. Removing excess water from the blood causes a decrease in the volume of fluid circulating through the blood vessels. This drop in fluid volume decreases the effort required by the heart to pump blood around the body. There are many conditions which may lead to an accumulation of fluid in the body (oedema). This medicine is commonly used in heart failure, where the pumping mechanism of the heart is less effective. It is used to relieve the symptoms of heart failure, such as the shortness of breath seen with fluid on the lungs. Hydroflumethiazide when used alone can cause low potassium levels in the blood (hypokalaemia). Spironolactone on the other hand prevents potassium loss, and is included in this preparation not only to improve its effectiveness but also to prevent excessive potassium loss.

It is recommended that individuals taking this medicine have their fluid and salt (electrolyte) balance monitored regularly.
If you experience any of the following symptoms while taking this medicine you should inform your doctor promptly, so that the amount of fluids and salts in your body can be checked: thirst, lethargy, confusion, weakness, drowsiness, muscle cramps, scanty production of urine, abnormal heart rhythm, seizures, nausea and vomiting.
As diuretics cause your kidneys to produce more urine, you may prefer to take this medicine in the morning rather than before going to bed, as this will reduce the likelihood of you needing to get up in the night to visit the toilet. Seek further advice from your doctor or pharmacist.

High levels of fats (lipids) such as cholesterol and triglycerides in the blood (hyperlipidaemia)

Long-term inflammation of skin and some internal organs (systemic lupus erythematosus)
Severe liver disease


Not to be used in


Allergy to medicines from the sulphonamide group, eg the antibiotic sulphamethoxazole
Allergy to thiazides
Failure of the kidneys to produce urine (anuria)
High levels of calcium in the blood (hypercalcaemia)
High levels of potassium in the blood (hyperkalaemia)
Inadequate production of natural steroid hormones by the adrenal glands (Addison's disease)
Severe and worsening kidney disease
Sudden decrease in kidney function (acute renal insufficiency)

Pregnancy and Breastfeeding

This medicine passes into breast milk. It is recommended that mothers should avoid using this medicine while breastfeeding. Seek medical advice from your doctor.


Side effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. A drop in blood pressure that occurs when going from lying down to sitting or standing, which results in dizziness and lightheadedness (postural hypotension)
Impotence
Fatigue
Blood disorders
Skin rashes
Disturbances of the gut such as diarrhoea, constipation, nausea, vomiting or abdominal pain
High blood glucose level (hyperglycaemia)
High blood uric acid level (hyperuricaemia) which can cause kidney problems and gout
Abnormal enlargement of breasts in men (gynaecomastia)
Irregular menstrual cycle
Disturbances in the levels of chemical components (electrolytes) in the blood

How can this medicine affect other medicines?

When taken together with ACE inhibitors or potassium salts there is an increased risk of high blood potassium levels (hyperkalaemia) and the combination should be avoided.Spironolactone increases the levels of cardiac glycosides such as digoxin in the blood and should be monitored regularly. If a decrease in blood potassium levels (hypokalaemia) occurs with hydroflumethiazide, the adverse effects of cardiac glycosides such as digoxin may be increased.Spironolactone decreases the ulcer healing effects of carbenoxolone.Hydroflumethiazide reduces the ability of the body to remove lithium. As this can give rise to adverse effects, this medicine should not be taken with lithium.Hydroflumethiazide when taken with large doses of Vitamin D and/or calcium may cause an increase in the levels of calcium in the blood. This may lead to adverse effects.The effects of hydroflumethiazide may be reduced by indomethacin.When taken together with other medicines such as the corticosteroids and carbenoxolone, which can cause a decrease in blood potassium levels (hypokalaemia), there may be an increased risk of developing hypokalaemia. Potassium supplements may be required.
